CAREPROX Adult Day Program Center, offers services and a range of individual or group activities designed according to every participants service plans to enhance their social, emotional, physical and creative needs. Some of our unique services/activities include:
Personal Care – assisting participants eating, bathing, cleaning, and toileting.
Daily Living Skills – encouraging participants to improve their daily living skills like cleaning, hygiene and hanging of clothes.

Activities Like:
Scheduled Outing Activities – Visiting a local recreational center, going to a bowling alley, playing basketball, taking a walk and many more.
Creative Activities – Art/Computer projects, crafts, as well as in-center music concerts and performances.
Indoor Physical Exercises – Range of Motion exercises, Indoor Croquet and Upright Cycle exercises.
Extracurricular Activities – Social events, such as Volunteering, Job fair and community events. Integrating our Participants within the community.

Supported Community Connections (SCC), activities may be provided one on one or in a small group setting.
Supporting the abilities and skills necessary to enable the participant to access typical activities and functions of community life such as those chosen by the general population, including community education or training, and volunteer activities.
Providing a wide variety of opportunities to facilitate and build relationships and natural supports in the community, while utilizing the community as a learning environment to provide services and supports as identified in the Service Plan.
Promoting safety and independence, develop skills for participation in the community, and work toward personal goals.

Community Connector (CC)
Assisting with accessing community activities and resources for participants. Service will require oversight by the Support Services Coordinator or the participant’s Behavior Therapist.

Assisting with understanding complicated health and safety information, completing and reviewing safety assessments.
Accompanying consumers to medical appointments to aid in communication with providers, understanding medical information, and assisting the individual with a follow-up.
Meet with individuals one on one to instruct, model, advise, or provide experiences to assist with full participation in their communities.
Assisting individuals with interviewing potential providers, and with participation on private or public boards, advisory groups and commissions.
Mentors can assist individuals with finding resources in their families or in the community.
